Identify Verification
This form is used to submit identity verification documents to resolve information discrepancy.
Jefferson Community and Technical College requires the confirmation of correct date of birth or social security number in situations where a student has submitted information that conflicts with a previous submission or matches another student in our system.
Documents accepted for verification include:
- Social Security Number
- Social Security Card
- Date of Birth
- Birth Certificate
- Valid Government Issued Photo ID (passport included)
Please note, you may also submit documents in person during normal business hours at the Jefferson Education Center, Suite 200 (Admissions Office) at the following physical address:
