Physical Therapist Assistant Program Pre-Admission Form

APPLICATION INSTRUCTIONS AND DEMOGRAPHIC INFORMATION

This is the official form to apply to the Physical Therapist Assistant Program at Jefferson Community and Technical College. Students must verify their attendance during the preadmission conference to be given access to this form. This form and all of its supplemental documents must be submitted to the college by the April 1st deadline to qualify for admission.

In order for the student's application to be complete, additional documents must be submitted to the Jefferson Admissions Department that support the information provided here. These documents include: application to Jefferson Community and Technical College (if not enrolled for a spring course within a KCTCS school), official ACT Composite Score Report, official transcripts from any previous college(s) attended, and an official high school transcript. International students wanting to have prior college credit accepted by JCTC must refer to the list of acceptable international transcript evaluators that are provided on the college's website.

PRIOR TO SUBMITTING THIS FORM

Be advised that this form may take 30-60 minutes to complete. You will need to have all of your academic records, contact information for work supervisors, etc., and ACT composite score reports available.

Please be sure that you have submitted a complete application to JCTC (following all requirements set forth by the college) or are currently enrolled at another KCTCS institution for the spring semester. You must have an active KCTCS ID number to complete this form.
