This form is reviewed by our Wellness & Counseling department at Jefferson during normal operating hours. If you do not hear back within 1-3 days, you can follow up by contacting us at JF-Counseling@kctcs.edu. If this is an emergency situation involves an imminent risk of harm to self or others, contact 911 prior to filling out this referral form.
Winter Break Notice
December 14 is the LAST day that intake requests will be processed for the Fall 2025 semester. Any requests submitted after this date will be put on hold until the college reopens for the Spring semester on January 5, 2026.
If you have any questions or concerns, please email jf-counseling@kctcs.edu. Emails sent after business closes on December 19 will not be addressed until the college reopens on January 5.
