The Academic Plan of Action Memorandum of Understanding is used to satisfy the "SAP - Plan of Action" item in your Student Self-Service. Only complete this form if you have been directed to by the Financial Aid Office. Please allow 2-3 business days for the completed form to be reflected on your student account.
