Forms

Students who wish to receive Form I-20 from Jefferson Community and Technical College must complete all admissions requirements outlined in the Admission Process for International Students by the designated deadline.

If all requirements are completed on time, the International Admissions Office will issue Form I-20, which confirms your acceptance to study at Jefferson. You will use this I-20 to apply for F-1 student status, either at a U.S. Embassy or Consulate abroad or, in some cases, from within the U.S.

You must always keep your I-20 up to date. Any changes to the following information must be reported to the International Admissions Officer within 10 days:

  • U.S. address
  • Home country address
  • Major
  • Financial support
  • Legal name
  • Expected program completion date

If you have a spouse or child under the age of 18 who will accompany you to the U.S., each dependent must have their own Form I-20. They will use their I-20 to apply for an F-2 visa, based on your F-1 status. Their information must also be kept current.

When you receive your I-20, you must read page 2 carefully, then sign page 1 to confirm that you understand the rules and responsibilities associated with this document. Additionally, the Professional / Designated School Official (P/DSO) at Jefferson must have signed the I-20 in blue ink for it to be considered valid.

You must complete U.S. government forms for the U.S. Embassy when you apply for your visa.
