If you have never submitted an Education Claim to VA Complete VA Form 22-1990 with the Veterans Affairs Office on campus or call 1-888-442-4551 and have the DVA mail you a form.

If you choose to complete your form through the Veterans Affairs Office on your campus, please bring the following with you:

• DD Form 214,
• Copies of Orders to Active Duty and
• Annotate CHAPTER 1607 on ALL supporting documents.

If you ve USED a VA Education program previously NO new application is required, but you should submit and have available documents such as:

• DD Form 214,
• Copies of Orders to Active Duty,
• Complete VA Form 22-1995 and
• Annotate CHAPTER 1607 on ALL supporting documents
