Student Request for ARC Services
NOTICE: Starting in Fall 2025, returning students will be required to submit a Renewal of ARC Services each semester before a new accommodation letter will be created. This is to inform us that you would like to continue receiving accommodations through the ARC.
To Receive Accommodations through the ARC
- Submit the request form below
- Submit any additional disability documentation you have to jf-arc-office@kctcs.edu
- Meet with an ARC coordinator to get your accommodations approved.
Once you submit this form, an ARC coordinator will reach out via email to schedule an intake meeting. If you do not receive a scheduling request within 1-3 business days, please feel free to follow up with us at jf-arc-office@kctcs.edu.
Students must meet with a coordinator before they will receive any accommodations.
Request for ARC Services Form
A request form MUST be submitted, and accommodations MUST be approved to receive accommodations. Please view Documentation before submitting this request. Deaf and Hard of Hearing students MUST attach an audiology report or doctor's note stating deafness. If you change your schedule, please contact the Access*Ability Resource Center immediately so the correct instructor(s) will be notified.

Students requesting accommodations must be the individual to sign and submit the form.
If documentation is unavailable at this time, attach a note stating this. Further documentation can be submitted to jf-arc-office@kctcs.edu.
